Participant Kit Order Form
- North American and International orders: only LivingWorks trainers or facilitators or known agents of trainers or facilitators can order materials
- Australia Orders: trainers must order through the member site. Forms are available in each program specific training center.
- Allow at least three weeks for delivery unless special shipping requested.
- Shipping and handling fees apply to all orders as well as sales tax where applicable. Fees will be determined according to shipping specifications and will be added to your invoice.
- Please complete all ship-to and bill- to fields in full. If you are requesting a quote and do not have all details, enter N/A.
- International orders may be subject to additional duties and taxes imposed by the destination country. We cannot guarantee if these charges will be billed or not and it is the responsibility of the recipient to pay these fees should they occur. For more information, please check the importation laws and regulations for your country.
- Materials are non-refundable. Please count all material immediately after delivery. Missing items cannot be replaced beyond 30 days after delivery.

- LivingWorks requires prepayment, a purchases order or a contract before materials can be shipped. A sales order with payment information will be sent to the email you provide when completing this form.
Prices for the following will be:
- ASIST Kits: $40.75
- safeTALK Kits: $10.15
- A $20.00 priority handling fee will apply to all rush orders (those with less than three weeks’ lead time).
